After many years of helping patients set and achieve goals related to their health, here are the clinical team’s top tips for success.
Identify your personal objectives
- Recognising your unique needs and aspirations is crucial to crafting goals that resonate with you.
- Reflect on what motivates you to embark on this journey towards better health.
- Consider the aspects of health and well-being that are most meaningful to you.
- Evaluate your current situation and focus on areas that may require improvement or change.
- Determine which goals are most critical to your overall well-being and focus on them.
Break down goals
- Sometimes our long term goals are ambitious, and this is motivating! However, if they are going to take a little while to achieve, it can help to break them down into smaller milestones so we don’t get fatigued along the way.
- Celebrate each milestone achieved to maintain motivation.
Create actionable plans
- Once your goals are defined, it's time to draft some actionable plans, that is things you can do or steps you can take to achieve the goal.
- Consider the resources and support you may need.
- Anticipate what barriers may prevent you from succeeding and plan ways to navigate these.
Track your progress!
- This is very important! We know that goal setting is most successful when paired with self monitoring.
- If your goals are around weight loss, reducing blood pressure or waist circumference, you can track these in the Me section of the Roczen App.
- Use a calendar to tick off days when you have achieved your goals.
- Share achievements and challenges with your mentor, the clinical team or other patients in our official Facebook group.
- Try to attend the 4 week reviews with our clinical team so we can provide you feedback and review your progress.
Keep motivation high
- Acknowledge your efforts and celebrate every step forward, no matter how small.
- Embrace setbacks as learning opportunities and use them as key information to refine your plan.
- Treat yourself to rewards when you achieve milestones, therefore reinforcing positive behaviour.
- Practice self-compassion and be kind to yourself during challenging times.

Build a support system
- Within Roczen, you have access to a fantastic support system, whether that’s other patients, your Mentor or the clinical team.
- Engage with the community (Strava, Facebook Group) for shared experiences and encouragement.
- Tell your friends and family about your goals, explain why they are important to you and ask for their help in keeping you on track.
Adjust when necessary
- Regularly evaluate your progress and assess if your goals need adjustments - your clinician will help you with this!
- Be open to modifying your strategies as you learn and grow on your journey.
- Stay committed to your objectives, but balance this with other life priorities, don’t be afraid to hit pause when you need to.
Maintaining Long-Term Success
- Adopt sustainable lifestyle changes for weight loss and diabetes management, remembering that these are long term conditions and they require a long term approach.
- Set new goals to keep progressing and growing to ensure they align with your current priorities.
